Your options to request to use our service
There are different options for you to request to use our service yourself. You can call us, you can email us or you can complete a request form and send it to us. If you're requesting the service on behalf of someone else, you will need to complete the form for representatives.
You can request to use our service yourself
You can request to use our service yourself now by completing this form. By doing so, you are agreeing to us contacting ACC to see whether they agree to Talk – Meet – Resolve with you. Please be aware that you are not lodging a review by completing this form.
If you're an advocate or an informal representative (family/whānau member), then please use the representative form below.

Request Talk – Meet – Resolve for someone else by completing our Request Form
You can request to use our service on behalf of someone else by completing this representatives form. By doing so, you are agreeing to us contacting ACC to see whether they agree to Talk – Meet – Resolve with that person and you as a representative. Please be aware that you are not lodging a review by completing this form.
If you're an advocate or an informal representative (family/whānau member), you will also need to complete an authority to act before we can process your request.
